심장수술편

 

CABG : saphenous vein grafts, internal mammary artery graft,, vein 의 경우 valve 있고 artery 보다는 예후 떨어지며 internal mammary artery 내흉동맥 은 보통 심장에 크게 영향미치는 동맥이 아니므로 이걸 자주 사용한다고 한다. artery라 예후도 굳,, 위치에 따라 RIMA, LIMA 로 나뉜다.

 

3 vein : RCA, LAD, LCX 

 

CABG post intervention,, Potasium 4 밑으로 떨어지지 않도록 fluid care 해줘야 한다.

 

The tansplanted heart is denervated and unresponsive to vagal (미주신경)stimulation(자극) → 환자는 흉통이 있어도 신경잘려있어 모른다. verapamil 같은 약 사용해도 약효없음.

transplate 시 rejection effect 로 Ex) EF 60% → 10% 뚝 떨어지기 가능

Rejection effect 시 endomyocardial biopsy로 vein → 우심방 → 우심실 biopsy,, rejection 반응 본다. 

1주 한번, 2주 한번, 1달 한번,, 2달, 6개월,, f/u course 정해져 있다. the client requires lifetime 평 생  immunosuppressive therapy. 

vegal stimulation 에 반응하지 않기에 exercise stress에도 일반 심장만큼 확 오르거나 하지 않는다.
